, and we're looking for a
Physical Therapist
to serve as our
Director of Outpatient Therapy
.
This is a hands-on clinical leadership role for a PT who wants to continue treating patients while taking the next step in their career. You'll maintain an active patient caseload while serving as the primary Blue Stone Therapy leader on-site at Charter House, supporting the therapy team and helping shape our growing outpatient program.

What You’ll Do
coordinate the day-to-day operations of the therapy program. You'll serve as a resource for the therapy team and help create a collaborative, patient-focused environment.
Responsibilities include:
Providing Physical Therapy services in accordance with professional standards and physician orders
Performing patient evaluations and developing individualized treatment plans
Supporting day-to-day therapy operations, scheduling, caseloads, and workflow
Serving as the primary Blue Stone Therapy leader on-site
Supporting onboarding, mentoring, and development of therapy team members
Building collaborative relationships with
Charter House
leadership and interdisciplinary team members
Monitoring documentation, regulatory, and payer requirements
Helping identify opportunities to grow and enhance therapy services
Supporting quality outcomes and a positive patient experience
Requirements
Qualifications
Graduate of an accredited Physical Therapy program
Current Minnesota Physical Therapist license or eligibility for licensure
