Key Responsibilities – What You’ll Do
Deliver Patient-Centered Care
- Conduct assessments, provide treatments, and ensure strong follow-up care.
Collaborate Across Teams
- Partner with providers and healthcare staff to create effective care plans.
Educate & Empower
- Support patients and families with knowledge on conditions, treatments, and prevention.
Administer Medications
- Safely deliver medications and treatments per protocols.
Maintain Records
- Accurately document patient care and changes in condition.
Drive Quality Improvement
- Participate in initiatives that enhance care and outcomes.
Provide Inclusive Care
- Deliver respectful, culturally competent care to diverse populations

Qualifications: Required:
- Nursing degree from an accredited nursing program
- Current LPN or RN license in state of practice
Valid driver’s license
Preferred
2 yrs of experience of nursing experience
2 yrs of experience in primary care
