Ealing Short Term Rehabilitation - Band 5 Community Occupational therapist
The Ealing Short Term Rehabilitation (STR) team provides therapy-led rehabilitation to help patients improve their abilities and achieve theirrehabilitation goals, through therapy led care. We focus on achieving goals and enabling people to regain independence and social integrationeffectively and efficiently after discharge from acute hospitals and community services in the borough. Therapists and rehabilitation assistantsoffer support for a period of up to six weeks, usually less if goals are achieved or care can be delivered in a different way.
To provide specialised rehabilitation assessment, education, advice and treatment to adult patients in the community, who have complex health and social care needs.
