Speech-Language Pathologist (SLP) – PRN to join our therapy team in

As an SLP, you will provide skilled speech-language pathology services to patients with a variety of communication, cognitive and swallowing needs.
Responsibilities include: Evaluate patients and develop individualized treatment plans
Provide skilled therapy for speech, language, communication, cognition and swallowing
Monitor patient progress and modify treatment as appropriate
Complete timely and accurate clinical documentation
Communicate with patients, families, physicians and interdisciplinary team members
Participate in care planning and discharge planning
Educate patients, caregivers and facility staff
Provide in-service training and support restorative/functional maintenance programs when appropriate
Help patients maximize their independence and quality of life

Strong clinical judgment and communication skills

Ability to work independently while collaborating with an interdisciplinary team

Previous healthcare, SNF or post-acute rehabilitation experience is a plus
