HASP PCN is a network of five GP practices across Hull. The PCN Pharmacist will work as part of the PCN overlay team providing services to all member practices.
Working alongside PCN and practice teams to provide pharmacy services to the PCN population. Please see Job Description for further details.
HASP PCN is a network of
five GP practices in Hull serving a population of approx. 35,000 patients. The
Primary Care Pharmacist (Independent Prescriber) will work as part of the
multidisciplinary team (MDT) within the Primary Care Network to deliver
high-quality, patient-centred clinical pharmacy services. The post holder will
undertake structured medication reviews, support the management of long-term
conditions, provide prescribing support, contribute to Quality and Outcomes
Framework (QOF) achievement and assist in the delivery of PCN Directed Enhanced
Service (DES) requirements.
