Lead a Service That Helps People Flourish
As the Health and Wellbeing Practitioner, you will play a central role in ensuring WE: Flourish delivers safe, effective and person-centred support across Erewash. You will provide leadership and oversight of the service, supporting referral management, triage, quality assurance, multidisciplinary working, complex case support and service development.
Erewash Health Partnership is a
single GP partnership providing core primary care services and support services
across its constituent practices. EHP is also the prime provider for services
delivered on behalf of Erewash Primary Care Network.
