Hypertension Coach

Salary: £27,976 a year

We are looking for a motivated
and compassionate Hypertension Health Coach to support individuals living with
hypertension in making sustainable lifestyle changes that improve their hypertension.
You will work directly with participants across Bath and Northeast Somerset to
facilitate positive behaviour change using evidence-based approaches, tailoring
support to individual needs. You will work in collaboration with GP surgeries,
community pharmacies, and the Voluntary, Community, Faith and Social Enterprise
(VCFSE) sector to establish and maintain clear referral pathways, ensuring
eligible can easily access the service.
