is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide one-on-one care for a client in the home setting. This role includes skilled nursing services and transportation assistance during schedule shifts.

Responsibilities:
Provide direct nursing care in accordance with physician orders and the care plan
Administer medications and treatments
Monitor and document changes in client condition
Communicate with supervisor regarding client needs and concerns
Complete accurate and timely documentation
Provide transportation assistance as needed

Requirements
Active LPN license in good standing
Valid driver’s license, car insurance, and reliable transportation
Ability to lift, transfer, and assist up to 40 lbs
Ability to uphold client confidentiality
Ability to work independently in a home setting
Excellent communication and interpersonal skills
