If you have a passion for rehabilitation, thrive working within multidisciplinary teams and have innovative ideas to develop and deliver personalised care plans that support people to maintain their health and wellbeing, to promote hospital admission avoidance. You will need excellent communication skills and commitment to high quality patient care alongside flexibility and resilience to meet the challenges of community working.
