Salary: £39959.00 to £48117.00
An opportunity has arisen for a Band 6 Registered Mental Health/Learning Disability Nurses, Social Workers or Occupational Therapists to join the North Home-Based Treatment Team.
The post holder will undertake collaborative care planning with service users and their carers.
Undertake risk assessment and record risks in line with the trust policy
Undertake shift co-ordination when allocated to do so.
Participate in Multi-disciplinary Team(MDT) and agree plans with patients in relation to their care.
Maintain their knowledge of evidence based home treatment interventions and deliver these interventions in line with service use care.
Undertake collaborative care planning with services users and their carers.
Undertake gatekeeping assessments to ensure service users are treated in the least restrictive environment.
Undertake allocation of shift co-ordination.
