Salary: £25760.00 to £27476.00
We are looking for an enthusiastic person to boost our existing Extended Primary Care Team. Our services cover a wide geographical area including Fareham & Gosport, Portsmouth, Havant, Waterlooville and Petersfield and Bordon. This role is for 30 hrs per week.
Health Connectors provide a service which helps members of the community access support to better self-manage their health and wellbeing and long-term health conditions.
