About the role
Salary: £29000.00 to £35000.00
Are you motivated with excellent organisation and communication
skills? Do you want to make a difference
to support people to live longer and healthier lives? Do you have an interest in frailty?
The
post holder will support older people living with frailty to live longer,
healthier lives. They will work closely with Liberty PCNs five GP practices,
the wider PCN team, the local authority, including social care and housing, and
Voluntary Community and Social Enterprise
organisations to ensure individuals receive appropriate, coordinated
support.
