As a Community Nurse Band 5, you will deliver high-quality, person centred nursing care to patients in their own homes, working in line with national and local policies, professional standards, and the NMC Code. You will work closely with patients, families, GPs, and multidisciplinary colleagues to assess needs, plan and deliver care, promote independence, and support positive health outcomes. This rewarding role offers the opportunity to work autonomously while being part of a supportive community team committed to delivering safe, effective, and evidence-based care.
