Package Description: Patient Flow and Discharge Team An exciting opportunity has arisen to join the Patient Flow and Discharge Team at Great Western Hospital. The post holder will be responsible for supporting safe, effective and timely discharge planning across inpatient services, ensuring patients are assessed holistically and directed to the most appropriate discharge pathway. Working collaboratively with consultants, ward teams, community providers, social care and voluntary sector organisations, the successful candidate will facilitate patient flow and contribute to the delivery of high-quality, person-centred care. The role forms part of a multidisciplinary seven-day service focused on improving patient experience, reducing delays and supporting positive healthcare outcomes. Responsibilities include undertaking holistic assessments and contributing to discharge planning using evidence-based practices. The post holder will liaise with acute, community, social care, and voluntary sector services to ensure continuity of care. Clinical leadership and support to junior team members, managing complex patient cases, and supporting operational service delivery are key aspects. The role requires experience in discharge planning, effective communication, negotiation, and conflict resolution skills, and knowledge of the Care Act, Mental Capacity Act, and safeguarding processes.
