Salary: £50 an hour
Advanced Nurse Practitioner / Advanced Clinical Practitioner: Extended Access Hub, Belmont Health Centre, Harrow
Assess, diagnose and treat patients aged 2 years and over presenting with minor illness and other undifferentiated, acute conditions, in 15-minute appointments.
Review and manage patients with long-term conditions, as clinically appropriate within the Extended Access service.
Work autonomously within your professional scope of practice, using sound clinical reasoning to manage risk and make safe decisions.
Prescribe as an independent prescriber, in line with local formularies, national guidance and safe prescribing standards.
Request and interpret investigations where clinically appropriate, and act on the results.
Safety-net appropriately, and escalate or refer on to the right urgent, emergency or onward care when needed.
Keep accurate, timely and contemporaneous clinical records on the practice clinical system (EMIS).
Follow PCN and practice policies, including safeguarding, infection control, information governance and clinical governance.
Work as part of the wider PCN team, communicating well with colleagues across the Extended Access Hub and member practices.
Take part in audit, learning and service improvement where appropriate, and keep your own professional development and revalidation up to date.
