We are seeking a dynamic, patient-focused Advanced Practitioner (AP) with a specialist interest in rehabilitation to join the MDT on Oxford and Cambridge Ward at Herts & Essex Hospital.
Carry out weekly ward round alongside the ward GP and Speciality doctor.
Prescribe and review medication as per required/appropriate for patients on the unit.
Interpret/request blood tests where appropriate for patients on the unit.
Interpret/request other investigations.
Develop medical treatment plans for patients on the unit.
Provide supervision and support for the nursing team on the unit.
Coach staff both internal to HCT and external seeking advice on patients' suitability for admission to the unit and liaise with the acute trusts.
Support in the development/training of ACPs across HCT.
Work alongside the project development team in HCT.
Communicate with other HCT Inpatient units to ensure appropriate medical/ACP cover across all three sites.
The post holder will apply advanced clinical expertise, levels of judgement, discretion and decision making. Will act as an autonomous practitioner working independently and will exercise expert clinical practice in assessment, diagnosis, prescribing and development of medical treatment plans for patients with neurological needs, complex presentations and multi pathologies.
