About the role
Salary: £47951.00 to £56863.00
We are looking to recruit an experienced, motivated individual to join our complex discharge team. You will be predominantly based at Charing Cross Hospital site but will be expected to work across site as required.
To deliver high standards of patient care and pathway management to reduce delayed transfers of care across the trust, ensuring a safe, patient centred discharge.
To undertake / coordinate the assessment of patients' needs.
To act as the SMH Discharge Hub screener on a rotational basis.
To deputise for the Discharge Team Manager as required, attending Trust meetings where relevant to the role.
To assist in identifying educational needs of staff in other services and contribute to developing and delivering educational opportunities to staff within the hospital and externally.
To line manage, provide clinical supervision and support and undertake PDR's with allocated junior staff.
To work within the Trust's Discharge Lounge when required ensuring patients are transferred home safely and to maximum overall utilisation of the unit.
The post holder is required to work on the weekend and bank holiday rota as part of their contracted hours.
