Home Health Admissions RN
to help patients and their families transition into care at home.
As our Admissions RN, you'll complete initial assessments, determine eligibility, explain services, and coordinate with the care team to ensure a smooth start of care.
Salary: 85,000-95k,000 (Based on Experience)
Schedule
Friday, Saturday, Sunday, and Monday | Four 10-hour shifts (40 hours per week)

Responsibilities of the Home Health Admissions Nurse
As a Home Health Admissions RN, you'll be the first clinical point of contact for patients beginning their care at home. You'll complete thorough assessments to determine eligibility, explain our services, and guide patients and their families through the admission process with clarity and confidence. You'll work closely with physicians and the interdisciplinary team to gather medical information and ensure a smooth, timely start of care. Strong documentation and communication skills are essential, as you'll play a key role in setting the tone for high-quality, patient-centered home health care.
Qualifications of the Home Health Admissions Nurse
- Must be a Registered Nurse with a current license to practice in the Commonwealth of Virginia
- At least one year of home health experience is preferred but not required.
- Proficiency in using electronic medical record (EMR) systems.
