We are currently looking for a part time Band 6 Memory Nurse in the East/North East locality for 28.5 hours a week, working days are negotiable over Monday-Friday. This locality is based near Chapeltown in Leeds LS7 and covers the East/ North East part of the city. We are looking for
a
committed and motivated practitioner who wants to consolidate their nursing practice in memory services and develop their skills, experience and knowledge. The successful applicant will be expected to demonstrate the capacity to work independently in the assessment for dementia process and offering post diagnostic support.
Undertake comprehensive assessments of individuals referred with memory concerns, including cognitive, mental health, physical health, and social care needs.
The service operates between Monday and Friday, 9am till 5pm.
