About the role

The Care Home Advanced Clinical Practitioner (ACP) will operate as
an autonomous, independent and highly
skilled clinician within the Primary Care Network. Who will manage the day-to-day
operations of the Care Home team which covers both reactive care and proactive
care for residents living within care home settings. The post holder will
predominantly provide rapid assessment and treatment of
unwell residents
and provide clinical and management support as required to the proactive side
of the team.
