Help turn digital ideas into changes that
primary care teams can use. As Programme Lead Digital Transformation, you
will support projects from initial use case through rollout, training and evaluation,
keeping delivery organised and helping colleagues adopt new ways of working.
This is a practical programme delivery position
with the chance to see ideas become part of everyday services. You will
coordinate projects and build the relationships, evidence and implementation
support needed to make change stick. Reporting to the Chief Operating Officer
and accountable to the Chief Clinical Information Officer, your
