The PCN Neighbourhood
Development and Service Improvement Coordinator will provide dedicated
operational capacity to support the development, coordination, and improvement
of services across Darlington PCNs 11 member practices and its developing
neighbourhood arrangements.
Working under the direction of
the PCN Business and Governance Manager, the postholder will coordinate agreed Neighbourhood and Integrated Neighbourhood Team (INT) workstreams, support
engagement between member practices and partner organisations, undertake
structured reviews of PCN-funded and commissioned services, and coordinate the
implementation and monitoring of agreed service improvements.
