About the role
Salary: £23.52 to £30.16
We have
an exciting opportunity for an Advanced Pharmacist in Nottingham University
Hospitals NHS Trust to join the Clinical Pharmacy team at Queens Medical
Centre. You will be responsible for delivering a high-quality pharmacy service
within your allocated clinical specialty, managing key service requirements and
supporting the wider directorate in the delivery of effective medicines-related
care. You will be responsible for contributing to service leadership,
supervising and developing junior pharmacy staff, and working collaboratively
across clinical areas. The role also involves participating in the planned
clinical rotation programme, 7-day shifted rota and bank holiday duties, with
rotations across Queens Medical Centre and Nottingham City Hospital according
to service requirements.
As an
Advanced Pharmacist you will be responsible for applying specialist
pharmaceutical knowledge to ensure medicines are prescribed and used safely,
rationally and effectively. You will be managing medication-related
requirements through medicines reconciliation, prescription review, assessment
of treatment suitability, and facilitating the appropriate use of patients own
medicines and self-administration. You will be supporting patients by providing
medicines counselling and ensuring appropriate medication is supplied for
inpatient treatment, discharge and outpatient care. You will also be
responsible for interpreting and requesting relevant clinical tests to monitor
drug therapy and making evidence-based recommendations to optimise treatment.
Working closely with medical, nursing and other healthcare professionals, you
will provide specialist advice on medicines, promote adherence to the hospital
formulary and national guidelines, and contribute to improved therapeutic
outcomes.
