Salary: £203.96 to £238.48
The post holder will provide high-quality, patient-centred acute frailty care within the Humber Virtual Frailty Ward, working closely with the Community Frailty Hub to avoid unnecessary hospital admissions and support safe care in patients usual place of residence. The role involves autonomous and multidisciplinary working to assess, treat, and manage patients with acute frailty-related deterioration, including falls, delirium, mobility decline, deconditioning, medication concerns, and exacerbations of long-term conditions.
We are seeking a motivated and experienced
clinician to join an excellent multidisciplinary team, providing high-quality,
patient-centred care to vulnerable and frail patients in the community. The
role focuses on clinical assessment, care coordination, and supporting patients
to remain safely at home wherever possible.
