Salary: £28392.00 to £31157.00
This is an exciting opportunity for 24.5hrs per week working with the Community Macmillan Healthcare Team. The post holder will work within and as part of the Macmillan Health Care team supporting and working with both the clinical team and the administrative team to promote and facilitate effective leadership for the Macmillan healthcare team and facilitating patients who are approaching the end of their lives to achieve their preferred place of care within the community.
The post holder is responsible for supervising and coordinating the MHCT Care staff and ensuring patient care is carried out according to the care plan and as delegated by the registered nurse.
