Salary: £49387.00 to £56515.00
Are you an experienced community nurse leader with a passion for service transformation, clinical excellence and patient flow?
Provide clinical and operational leadership for the Centralised Triage Service.
Develop and implement triage pathways, standard operating procedures and referral management processes.
Ensure referrals are clinically assessed and prioritised in line with service criteria and risk management frameworks.
Monitor demand, capacity and workforce to maximise service effectiveness.
Play an active role in service redesign and transformation projects relating to access, assessment and referral management.
Work closely with District Nursing Teams, senior leadership and partner organisations.
Use data and performance information to identify trends, risks and opportunities for improvement.
Support governance, quality assurance and patient safety initiatives.
Lead and develop staff through supervision, appraisal and professional development.
Act as a clinical expert resource for complex referrals, case discussions and escalation decisions.
