What You'll Do
As a Home Health RN, you will provide skilled nursing care to patients in their homes while working closely with physicians, patients, families, and the interdisciplinary care team.
Your responsibilities will include:
Conducting comprehensive patient assessments and reassessments
Developing, implementing, and updating individualized Plans of Care
Providing skilled nursing interventions based on each patient's clinical needs
Completing accurate and timely
OASIS assessments and clinical documentation
Educating patients and families regarding disease processes, medications, treatments, and plan of care
Monitoring patient progress and communicating changes in condition appropriately
Collaborating with physicians and the interdisciplinary care team
Promoting patient safety, quality outcomes, and continuity of care
Providing compassionate, professional care that allows patients to remain safely at home

Requirements
Graduate of an accredited Registered Nurse educational program
Current Registered Nurse (RN) license within the state of practice
Home Health/Oasis experience required
Current CPR certification
Excellent verbal, written, organizational, collaboration, multi-tasking and time-management skills
Strong initiative, follow through, process orientation, attention-to-detail
Reliable transportation, valid driver’s license and current automobile insurance
