Job Summary
We are seeking a compassionate and skilled
Registered Nurse (RN)
to join our team. This role involves planning, organizing, and coordinating home health services, with a strong emphasis on community health and patient education.
Key Responsibilities
- Develop and initiate individualized care plans in collaboration with patients and caregivers
- Deliver physician-ordered nursing services and implement preventive and rehabilitative procedures
- Educate patients and caregivers to promote health and independence
- Identify and implement discharge planning needs
- Coordinate necessary equipment and services
Supervise LVNs and HHAs
Qualifications
Active California RN license
Minimum 1 year of direct patient care experience (home health preferred)
Current CPR certification
Valid driver’s license and insured, reliable vehicle
