- Lexington Country Place is seeking a dependable and detail-oriented
- MDS LPN to join our nursing team.
- Shift Details: 8 hours per week
- The MDS LPN is responsible for coordinating and completing MDS assessments, supporting accurate clinical documentation, and working with the interdisciplinary team to ensure quality resident care and regulatory compliance.

Responsibilities
- Complete and coordinate MDS assessments accurately and within required deadlines.
- Review clinical records and documentation to support accurate MDS coding.
- Collaborate with nursing, therapy, dietary, social services, and other departments.
- Participate in care planning and interdisciplinary team meetings.
- Monitor resident changes and ensure appropriate documentation.
- Assist with Medicare/Medicaid documentation and regulatory requirements.
- Maintain compliance with applicable CMS, state, and facility requirements.
Qualifications
- Current LPN/LVN license in the state of employment.
- Previous long-term-care or skilled-nursing experience preferred.
- MDS experience preferred, but training may be available for the right candidate.
- Strong organizational, communication, and documentation skills.
- Detail-oriented with the ability to meet deadlines.
- Ability to work effectively as part of an interdisciplinary team.
