We are seeking a Clinical Pharmacist to join our team at Richmond Clinic, supporting prescribing activity in general practice with a significant role in medication reviews.
Conducting comprehensive medication reviews and optimising medication regimens for patients with complex medical needs.
Providing advice on medication management to patients, healthcare professionals, and the multidisciplinary team.
Supporting the management of chronic diseases through medication adjustments and monitoring after discussion with medical team.
Corresponding with secondary teams if clarification needed of prescribing plans.
