Full-Time Physical Therapist Assistant (PTA)
to join our growing Home Health team in Pueblo. You'll provide one-on-one therapy in patients' homes, helping them regain strength, mobility, independence, and confidence where it matters most.

What You'll Do
As a Home Health PTA, you'll work under the direction of a licensed Physical Therapist to provide individualized treatment to patients in their homes.
You'll:
Provide skilled physical therapy interventions according to the established plan of care
Help patients improve mobility, strength, balance, function, and independence
Monitor patient progress and communicate changes or concerns to the supervising Physical Therapist
Educate and support patients and caregivers throughout the treatment process
Complete accurate and timely clinical documentation
Collaborate with nurses, therapists, physicians, and other members of the care team

Requirements
Qualifications
Graduate of an accredited Physical Therapist Assistant program with an associate degree
Current Colorado PTA license or eligibility for Colorado licensure
Valid, unrestricted driver's license and reliable transportation
Strong communication and time-management skills
Ability to work independently while collaborating with an interdisciplinary team
Previous PTA experience preferred
Home Health experience is a plus, but candidates with transferable therapy experience are encouraged to apply
Flexibility to assist with weekend coverage when needed
