We are looking for qualified GPs, to deliver a hybrid model of Hub based face to face clinics and home visits to support our practices. The post-holders main duty will be to conduct face to face appointments within a hub for our practices and home visits to undertake consultations with patients. The home visits will be face to face appointments, visiting patients in their own homes, triaged by the patients practice GP.
