HOW YOU'LL MAKE AN IMPACT

Pod-based teams of ~4–6 who sit and work together closely, covering for one another (very low low call-ins!). A nurse typically supports
one or two providers.
Medical assistants handle rooming. No fixed patient census; workload varies with the provider's daily schedule, with responsibility spread across the pod.

Coordinates care related to ambulatory cardiac monitoring, cardiovascular imaging, diagnostic testing, and invasive cardiac procedures while ensuring timely follow-up of results and patient communication

Triage and assess patient symptoms including chest pain, palpitations, shortness of breath, syncope, edema, blood pressure concerns, and arrhythmia-related complaints.

Review diagnostic results including ECGs, echocardiograms, stress tests, cardiac monitors, laboratory results, and imaging studies; communicate findings and provider recommendations to patients.

Coordinate care for patients with cardiovascular conditions including coronary artery disease, heart failure, atrial fibrillation, hypertension, valvular disease, peripheral vascular disease, and dyslipidemia.

Provide comprehensive patient education regarding medications, disease management, cardiac risk reduction, lifestyle modifications, and treatment plans.

Manage medication refills, prior authorizations, and treatment adherence for cardiovascular medications, including anticoagulants and guideline-directed medical therapy.

Collaborate with cardiologists, advanced practice providers, medical assistants, diagnostic departments, and external providers to ensure timely and coordinated patient care.

Facilitate scheduling and follow-up for diagnostic and therapeutic procedures, including cardiac catheterizations, stress testing, echocardiography, cardiac CT, and ambulatory cardiac monitoring.

Monitor and respond to abnormal diagnostic results and patient-reported symptoms according to established clinical protocols.

Provide telephone triage and electronic patient portal support while utilizing clinical judgment to determine appropriate disposition and escalation of care.

Assist with chronic disease management and population health initiatives aimed at improving cardiovascular outcomes and reducing hospital readmissions.

Maintain accurate and timely documentation within the electronic medical record in accordance with organizational and regulatory requirements.

Participate in quality improvement initiatives, patient safety activities, and workflow optimization efforts within the cardiology practice.

The Registered Clinical Nurse (RN) is fundamental in achieving recognized world-class health care. Assumes direct and indirect responsibility for the patient's care environment. The primary responsibility is providing care through collaboration with health care team members and the autonomous application of the nursing process. Also provides leadership, mentoring and role modeling for other members of the health care team. Responsibilities include, but are not limited to, coordination and delegation of care in a safe, secure and clean environment.
