Poole Central Primary Care Network are
recruiting an experienced Paramedic or ANP / ACP to work in our Acute Home
Visiting Team on a fixed term 1 year contract.
The post holder will support on the day demands in General Practice by undertaking
home visits to facilitate timely intervention for patients with acute health
needs in the wider context of the patients health. This will involve a
holistic assessment of their medical needs. The role will require the
postholder to provide autonomous, advanced clinical assessment, diagnosis,
treatment and management of patients requiring urgent or acute care within the
community setting. This will require the post holder to
work closely with the PCN and Practice Teams, as well as Community and Hospital
based services and social care teams all serving the Poole Central population.
The post holder will be an experienced
Advanced Clinical Practitioner ACP, Advanced Nurse Practitioner ANP or Registered Paramedic Practitioner
who, acting within their professional boundaries, will provide advanced clinical assessment, diagnosis and management of patients presenting within the primary care and community setting.
