We are looking to recruit a
Young Persons Care Coordinator
to work
as part of our Social Prescribing Team. Social prescribing connects and signposts people to
practical and emotional support such as peer support groups, activities,
housing, finances and lots more. The Young Persons
Care Coordinator will support our 5-25-year-old patients.
Our ideal candidate would be committed to supporting the wellbeing of
young people. A passion for safeguarding and promoting the welfare of children
and young people is essential.
Help young people to manage
their health and wellbeing needs, answering their queries, signposting and
supporting them to access a variety of services that will help them.
