The PCN Neighbourhood Lead plays a pivotal role within the
Primary Care Network (PCN) helping to ensure that patients receive
well-organised, person-centred, and proactive care. Serving as the Strategic
Lead for the PCN for the development and delivery of the Sherborne Integrated Neighbourhood,
working together to provide best service without overlap.
v Identify patient cohorts, that align with the contractual obligations of the PCN and practices. Supporting PCN projects
Acting as a principal point of contact for internal and
external stakeholders, across health, social care and VCSE partners. Building
and sustaining strong, effective relationships that support neighbourhood
delivery and improved population health outcomes. Helping to facilitate an
effective multi-disciplinary across organisations, breaking down organisational
boundaries and promoting a collaborative neighbourhood delivery of services.
