Frailty Healthcare Assistant

We
are currently seeking a
Frailty Health Care Assistant (HCA)
to join our
friendly and supportive team at West Moors Village Surgery. Sadly, our current
HCA is retiring to enjoy the good life! We would expect candidates to have
experience as an HCA, although not necessarily in a frailty role, as more
specific training can be provided. The focus of the
role is ensuring the delivery of safe and effective care to our patient
population.

The
HCA role involves a variety of duties including Phlebotomy (venepuncture), vaccinations,
ECG's, clinical observations, urinalysis, long-term condition checks and health
checks. (This list is not exhaustive).

Working under
the direct supervision of the Lead Frailty GP & ACP and strictly in
accordance with specific Practice guidelines and protocols, the Frailty HCA
will assist the Practice clinical team in the provision and delivery of
prescribed programmes of patient care:
