Salary: £66274.00 to £73496.00
This is an exciting opportunity to join the Cancer Services team at Whittington Health and play a pivotal role in shaping and delivering personalised cancer care, improving the experience, outcomes and quality of life of people living with and beyond cancer across both hospital and community settings.
The post holder will oversee the development, delivery and evaluation of PCC interventions, including Holistic Needs Assessments (HNAs), Personalised Care and Support Plans (PCSPs), End of Treatment Summaries (EOTS), Personalised Stratified Follow-Up (PSFU) pathways and Health and Wellbeing initiatives.
Main duties
The post holder will represent Whittington Health within local partnership programmes and contribute to wider neighbourhood health priorities. They will play a key role in delivering sustainable improvements in patient experience, quality of life and outcomes for people affected by cancer, while supporting the future development and sustainability of personalised cancer care services.
