Your Health Notts is an innovative community health organisation delivering evidence-based, person-centred services to improve the wellbeing of Nottinghamshire residents. Our mission is to reduce health inequalities through prevention, early intervention, and community engagement. We work collaboratively across the NHS, local authorities and the VCSE sector to empower people to live longer, healthier lives.
The postholder will deliver engaging healthy ageing education sessions using the Older and Wiser framework, alongside strength and balance taster sessions that promote independence and falls prevention. Sessions will be tailored to community needs, with guest partners involved where appropriate, and will focus on building positive, trusting relationships while representing Your Health Notts in the community.
The Community Ageing Well Programme is a new initiative developed in partnership with the Nottingham & Nottinghamshire Integrated Care Board. The programme aims to support older adults to remain independent, confident and socially connected by embedding healthy ageing education and strength & balance taster sessions within existing community groups across South Nottinghamshire.
