Salary: £32.00 to £35.00

The post holder will provide clinical care within a team consisting of general practitioners, advanced healthcare professionals (ANPs, clinical pharmacist), practice nurses and Healthcare assistants.
Patient contact will be mainly via triage, on-call duties and home visits
Duties include, but not limited to daily duty triage, home visits, post-discharge reviews and minor illness management
Provide care for presenting patient from initial history taking clinical assessment, diagnosis, treatment, and evaluation of care.
You must demonstrate safe, clinical decision-making and expert care, including assessment and diagnosis skills for patients within the general practice.
