The postholder will be responsible for coordinating and supporting the delivery of Middleton Integrated Healthcareservices and projects, ensuring effective use of digital systems, accurate patient and service information, and clear communication between practices, patients, care homes and external organisations.
Care Coordination & Care Home Support -
Coordinate care home patient lists, coding and alignment with weekly ward rounds; support care home vaccination programmes, including flu and COVID vaccinations, consents and care home planning; liaise with care homes, practices, pharmacy and other relevant services and support the coordination and delivery of Middleton Integrated Healthcare care initiatives.
