This is a unique opportunity to provide outpatient therapy within an established senior living community connected to one of Rochester's most recognized healthcare environments. As a Blue Stone Therapy Physical Therapist at Charter House, you'll work directly with residents to help them maintain mobility, independence, and quality of life.

What You'll Do
As a Physical Therapist, you will evaluate and treat patients with a variety of orthopedic, neurological, and medical conditions while helping them improve mobility, function, independence, and overall quality of life.
Responsibilities include:
Performing comprehensive patient evaluations and assessments
Developing individualized treatment plans and goals
Delivering evidence-based interventions focused on functional outcomes
Collaborating with patients, families, physicians, and healthcare team members
Educating patients on treatment plans, injury prevention, and wellness strategies
Maintaining timely and accurate documentation
Contributing to a positive patient experience and strong clinical culture
Requirements
Qualifications
Graduate of an accredited Physical Therapy program
Current Physical Therapist license or eligibility for licensure in Minnesota
Strong communication and interpersonal skills
Passion for delivering exceptional patient care
