An exciting
opportunity has arisen within Leigh Primary Care Network (PCN) to recruit a qualified / experienced Care Coordinators to join our team.The job requires a caring, dedicated, reliable, person-focused candidate
who enjoys working with a wide range of people. They will have good
organisational and time management skills, have data collection abilities and
be able to work on new projects. They will be highly motivated and proactive
with a flexible attitude, keen to work and learn as part of a team and
committed to providing people, their families and carers with high quality
support.
Work with people, their families & carers, to improve people's
understanding of their condition. Offer support to develop & review
personalised care & support plans to manage their needs & achieve
better healthcare outcomes.
The successful candidate will be based in Leigh PCN comprising of 10 GP
surgeries. We are looking to recruit two Care Coordinator to join our PCN team.
