Salary: £25000.00 to £28000.00
The
purpose of this dual role is to work as a Care Coordinator/Admin Assistant within the Primary Care Network. Supporting the NNHIP Project, Care Coordinating Team and Prescription Hub, for our five constituent practices within Connexus PCN: Birchwood
Medical Practice (Brislington), Nightingale Valley Practice (Brislington),
Priory Surgery (Knowle), Stockwood Medical Centre (Stockwood) and Wells Road
Surgery (Totterdown).
The role of the CareCoordinator/Admin Assistantin the Primary Care Network is to support our multi-disciplinary clinical team to ensure that our patients, particularly those who are vulnerable or have complex needs are able to access services which meet their needs.
