This job opportunity entails working hours from Monday to Friday, 9 am to 5 pm, with occasional weekend work required. As a Physical Health Nurse, you will lead and coordinate all aspects of the physical health needs of service users, fostering teamwork with multidisciplinary teams such as GPs and SALTs. This position is based at Castleholme Lodge, a 20-bed care home in Huddersfield, which is part of a cluster of Elysium services. Your responsibilities include developing healthcare plans, leading testing and interventions for conditions, educating staff, and promoting physical well-being among both patients and staff.
