An exciting opportunity has arisen for a highly motivated individual to join our friendly team as a Care Co-Ordinator. If you are looking for a part-time role in a friendly rural location, we may have the ideal position for you. The position is for 20 hours per week, days and times to be confirmed at interview.
The introduction of Care Co-ordinators within the Wyre Bridge PCN is a significant opportunity. This is a role which will evolve along with new functions but will involve working with our practice team and wider multidisciplinary teams as a pivotal role in ensuring all patients receive the best possible care and services available.
