The Care
Coordinator provides a patient-centered coordination service, supporting
patients to access appropriate health, wellbeing, social and community
services. The role involves working collaboratively with clinical and
non-clinical teams to identify patient needs, coordinate appropriate support,
improve engagement with healthcare and contribute to reducing health
inequalities.
