Integrated Discharge at Imperial College Healthcare NHS Trust has as an exciting opportunity within the Charing Cross Integrated Discharge Team. We are looking for an experienced, self -motivated, passionate and dedicated individual to join our Hospital Discharge team within the acute setting.
To lead and manage the complex discharge team on a day-to-day basis, as part of the Integrated Discharge Leadership team, providing clinical leadership, support and guidance.
To take day-to-day responsibility for the leadership, delivery and management of the complex discharge service.
Plan and organise this efficiently and effectively with regard to patient managementand use of time.
To ensure that all staff within the team have appropriate support for their practice and continuing professional development through team meetings, review, supervision,appraisal, training and development.
To review and update the documentation relating to discharge planning to improve the quality accuracy and consistency of information recorded and to undertake annual documentation audits.
To provide education to clinical and nursing staff regarding the remit of the Discharge team.
The post holder is required to work out of hours (specific rota details held locally within departments).
