An exciting opportunity has arisen for a highly
motivated and enthusiastic Mental Health Practitioner to join the Care Home
Liaison Team
Our ideal candidate would be a
committed team player as well as being able to work independently,
demonstrating initiative and be passionate in promoting good quality care to
maximise independence. Within this role you will be responsible for assessing,
developing, implementing and evaluating care plans using a person-centred
approach. Within this role you will gain training, support, and guidance to
ensure you are working collaboratively with external agencies to provide the
best possible care for our service users.
To
act as care co-ordinator / lead professional within the memory diagnostic process
for service-users in North East Lincolnshire.
This involves being responsible for assessment, developing, implementing
and evaluating care plans using a person-centred approach.
