Salary: £32073.00 to £39043.00
Calling all Registered Nurses (Adult, Mental Health or LD)
or Allied Health Professionals registered with the HCPC who have an interest in
Adults Continuing Healthcare and Funded Nursing Care, including Fast track. The
Practitioner role is part of the All Age Continuing Care Team in the cluster
ICB of NHS Dorset, NHS Somerset, NHS Bath, NE Somerset, Swindon &
Wiltshire. The role operates Monday to Friday core hours, with no night shifts
or weekends, with a hybrid approach of office work, remote working and face to
face assessments with individuals across the cluster footprint. For an informal

This role is for a Practitioner, with a requirement for
cross cover across the Cluster. Although there will be a primary allocation to
a Place team, the expectation is that we will be moving towards one ICB and
